In addition to our two main facilities at Lakeview Drive and Oakland Avenue, specific Oaklawn programs are located throughout Elkhart County and include:

• Semi-independent housing for adults with long-term mental illness located at 2518 Oakland Avenue, 1701 Prairie Street, 1711 Prairie Street and Oakland Estates, all in Elkhart, as well as 209 Lakeview Drive, Goshen

• Housing for the Amish population who are receiving services from Oaklawn, including Rest Haven, located at 225 Lakeview Drive, Goshen; Pleasant Haven, located at 61230 State Road 15, Goshen; and Horizons of Hope, located at 10877 Goldenrod Drive, Middlebury

• An adolescent group home located at 2600 Oakland Avenue, Elkhart
