How Are We Doing?

We hope that our clients are helped by and satisfied with the care they receive at Oaklawn. We welcome your compliments! However, if you are dissatisfied with our services, you are encouraged to first take the issue to your therapist and/or treatment team. If this effort is not successful, the complaint may be put in writing on our "How Are We Doing?" form that is available in the lobbies of all of our offices and our patient advocate will contact you. Or you may call our patient advocate directly at (574) 533-1234, ext. 459. If all these efforts prove unsatisfactory, you have the option of contacting the Joint Commission by phone at 1-800-994-6610 or by email at complaint@jointcommission.org. Other consumer advocate contacts include the Indiana Department of Mental Health Consumer Hotline at 1-800-901-1133 and Indiana Protection and Advocacy Services at 1-800-622-4845.
