Meet Jennifer. Jennifer has a chronic, serious mental illness. Because of that, she’s also at a higher risk for a number of physical health problems. But thanks to an innovative integrated care program at Oaklawn, Jennifer is fighting the odds. She sees a primary care physician located at Oaklawn’s campus, and she has a dedicated healthcare advocate, who serves as a link between her physical and mental health treatment teams. Since enrolling in the Oaklawn program, Jennifer is managing her diabetes, has lost more than 50 pounds and joined a weekly walking group. Jennifer says the team approach to care is helping her to live a healthier life — the life she wants.
